Join Our Team as a Full-Time LVN/LPN in Long-Term Care and Snafu Settings

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in Long-Term Care and Snafu settings. As an LVN/LPN in this role, you will play a vital part in delivering high-quality patient care and ensuring the well-being of our residents.

Key Responsibilities:

• Assess, plan, implement, and evaluate patient care plans to ensure optimal health outcomes • Develop, implement, and evaluate individualized care plans for residents, including medication management and wound care • Conduct vital signs, take medical histories, and perform physical assessments to identify patient needs • Administer medications, vaccinations, and treatments as prescribed by physicians • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care • Educate patients, families, and caregivers on health promotion, disease prevention, and management of chronic conditions • Document patient information, progress, and outcomes in electronic medical records • Participate in quality improvement initiatives and contribute to the development of evidence-based practices

Requirements:

• Current licensure as an LVN or LPN in the state of [insert state] • 1+ year of experience in Long-Term Care or Snafu settings • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment • Basic life support certification (BLS) and CPR certification • Ability to lift, transfer, and reposition patients as needed
