Key Responsibilities

• Assist licensed therapists in implementing treatment plans and documenting patient progress.
• Support patient mobility, rehabilitation, and recovery activities under supervision.
• Collaborate with healthcare team to ensure optimal patient care.
• Maintain accurate patient records in compliance with facility and regulatory standards.

Required Qualifications

• Licensed or certified as a Physical Therapy Assistant.
• Minimum of 1 year of professional SNF experience.
• Strong understanding of rehabilitation techniques and patient care procedures.
• Excellent communication and teamwork skills.
• Ability to work effectively in a fast-paced environment.

Preferred Experience / Skills

• Experience working with diverse patient populations.
• Familiarity with electronic medical records (EMR) systems.
