Key Responsibilities

• Provide skilled physical therapy services to patients in a Skilled Nursing Facility (SNF).
• Assess patient needs, develop treatment plans, and implement therapy interventions.
• Monitor patient progress and adjust treatments accordingly.
• Collaborate with healthcare team to ensure optimal patient care.
• Document treatment plans and patient progress accurately.
• Comply with all regulatory and safety standards.

Required Qualifications

• Minimum of 1 year of professional SNF experience.
• Valid state licensure as a Physical Therapist.
• Demonstrated knowledge of rehabilitation techniques and protocols.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills

• Experience working in a fast-paced SNF environment.
• Bilingual skills or additional certifications are a plus.
