Key Responsibilities
• Provide skilled physical therapy services to patients in a Skilled Nursing Facility (SNF) setting.
• Assess patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to promote patients' physical recovery and mobility.
• Document patient progress and update treatment plans accordingly.
• Collaborate with interdisciplinary team members to ensure optimal patient care.
• Maintain compliance with all regulatory and safety standards.

Required Qualifications
• Valid license or certification as a Physical Therapist.
• Minimum of 1 year of professional experience in a Skilled Nursing Facility (SNF).
• Strong knowledge of physical therapy practices and protocols.
• Excellent communication and patient care skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills
• Experience working in a long-term care or SNF environment.
• Bilingual abilities are a plus.
