Job Title

Schedule / Job Type

• Full-time, night shifts (7pm-7am), rotating days of the week

Key Responsibilities

• Provide compassionate, holistic end-of-life care to patients in a home-like short-term setting
• Assess and manage symptoms with excellence in assessment and critical thinking
• Educate patients and caregivers on care goals, fostering confidence and understanding
• Deliver comprehensive physical, emotional, social, and spiritual support
• Collaborate with the team to ensure personalized patient care and meet individual needs
• Participate in on-call rotation (2 shifts/month), including holidays and weekends

Required Qualifications

• Registered Nurse license
• Minimum of 2 years of acute care experience in medical-surgical, ICU, ER, or other acute settings (hospice experience preferred)
• Strong communication skills and ability to work effectively in a team environment
• Willingness to participate in on-call, holiday, and weekend rotations

Preferred Experience / Skills

• Hospice care experience beneficial but not required
• Experience with symptom management and end-of-life care
• Ability to assess complex patient conditions and develop appropriate care plans
