Key Responsibilities

  • Provide skilled home health nursing care to patients in their homes
  • Conduct wound care and monitor vital signs
  • Educate patients on medications and treatments
  • Communicate patient status changes to supervising RN or MD
  • Manage pain assessments and interventions
  • Handle pharmacy calls and medication refill requests

Required Qualifications

  • At least 1 year of experience in skilled home health care, including wound care and patient teaching
  • Valid BLS certification
  • Knowledge of infection control protocols
  • Strong communication skills for updating clinical teams

Preferred Experience / Skills

  • Prior experience with patient education and medication management in a home health setting
