Key Responsibilities

• Provide therapeutic services to patients in a skilled nursing facility
• Assess patient needs and develop individualized treatment plans
• Deliver therapy sessions for short-term and long-term care patients
• Collaborate with multidisciplinary team to ensure quality patient care
• Document patient progress and update treatment plans accordingly
• Participate in weekend rotation, working approx. 1 weekend day per month

Required Qualifications

• Valid state licensure as a Physical Therapist
• Proven experience working with diverse patient populations, including LTC and short-term care
• Strong communication and collaboration skills
• Ability to work independently and as part of a team

Preferred Experience / Skills

• Experience working in a skilled nursing facility or long-term care environment
• Knowledge of current therapy practices and compliance standards
