Key Responsibilities
- Provide physical therapy services to clients in a skilled nursing facility setting
- Develop and implement customized treatment plans to improve patient mobility and function
- Document patient progress and maintain accurate health records
- Collaborate with healthcare team members to ensure quality patient care
- Ensure compliance with all regulatory and safety standards
Required Qualifications
- Must have at least 1 year of professional SNF (Skilled Nursing Facility) experience
- Valid license or certification as a Physical Therapist
- Strong clinical skills and knowledge of therapeutic techniques
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Experience working with diverse patient populations
- Familiarity with electronic health records (EHR) systems
