Job Title

Key Responsibilities
• Provide skilled home health nursing care to a caseload of patients, including wound care and patient education
• Monitor vital signs and assess patient pain levels to manage care effectively
• Communicate patient status changes promptly to supervising RN or MD
• Conduct medication and treatment teaching for patients and their caregivers
• Manage pharmacy calls and refill requests for prescribed medications
• Practice infection control protocols to ensure patient and staff safety

Required Qualifications
• Minimum of 1 year experience in skilled home health care, including wound care and patient education
• Current BLS certification
• Knowledge of infection control procedures
• Ability to communicate effectively with healthcare team and patients
• Valid nursing license in the state of Virginia

Preferred Experience / Skills
• Experience working autonomously in a home health setting
• Strong assessment and teaching skills
• Local candidates preferred
