Job Title

Key Responsibilities
• Provide nursing care to patients in long-term care or skilled nursing facility settings.
• Assist with patient assessments, medication administration, and care planning.
• Monitor patient conditions and document progress using electronic medical records.
• Collaborate with interdisciplinary team to ensure quality patient care.
• Adhere to facility policies, procedures, and safety standards.
• Assist with patient transfers and provide support for dementia and rehab patients.

Required Qualifications
• Valid LVN or LPN license in the state of South Dakota.
• Minimum of 1 year experience in a LTC or SNF setting preferred.
• Strong patient assessment and documentation skills.
• Ability to work independently and as part of a team.
• Basic life support (BLS) certification required.

Preferred Experience / Skills
• Experience with EMR systems, specifically PCC.
• Prior experience with dementia and rehab patient populations.
• Ability to work 12-hour night shifts effectively.
