• Full-time, Days, 4x10 Hour Shifts

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting.
• Assess patient needs, develop treatment plans, and implement therapies accordingly.
• Document patient progress using EPIC and Rover systems.
• Collaborate with the care team to ensure effective patient care and safety.
• Exhibit professionalism and adhere to clinical standards and facility policies.
• Maintain transportation to visit up to six patients daily within the designated radius.

Required Qualifications

• Valid Physical Therapist license in the state of Oregon or ability to obtain.
• Minimum of 1 year of experienced in skilled home health physical therapy.
• BCLS certification.
• Reliable transportation.
• COVID-19 vaccination series including booster.

Preferred Experience / Skills

• Experience working with EPIC and Rover charting platforms.
• Prior experience in home health therapy.
• Ability to work independently and manage a caseload efficiently.
