• Full-time, Day shifts, 8 hours per shift
Key Responsibilities
• Provide outpatient physical therapy services to adult and middle-aged patients within a home health setting
• Manage a caseload of approximately 5-6 patients per day, ensuring quality and patient-centered care
• Develop and implement individualized treatment plans based on patient diagnoses such as orthopedics, cardiac, neuro, pulmonary, and diabetes
• Supervise and collaborate with rehabilitation assistants and other team members to ensure continuous patient progress
• Maintain accurate documentation using the Epic charting system and adhere to all compliance standards
• Demonstrate strong organizational, time management, and communication skills to efficiently handle case load and coordinate care
Required Qualifications
• Valid State License to practice as a Physical Therapist in Illinois (or compact license accepted in certain states with proper documentation)
• BLS certification
• Minimum of 1 year of recent relevant experience in home health or outpatient therapy setting
• Ability to pass background screening and compliance requirements, including travel compliance and TB screening
Preferred Experience / Skills
• Prior experience in orthopedic, cardiac, neuro, or pulmonary therapy
• Strong case management skills and ability to work independently within a multidisciplinary team
• Experience with Epic charting system is preferred
• Flexibility to work within a team environment and adapt to occasional weekend and holiday shifts as per policy
