Full-time, Days shifts, 3 shifts per week, 12-hour shifts
Key Responsibilities
- Provide professional home health nursing care to patients in their homes
- Perform assessments, develop care plans, and coordinate with healthcare teams
- Utilize OASIS documentation and ensure compliance with all regulatory requirements
- Manage case loads independently, delivering tailored patient care
- Maintain accurate and detailed patient records and documentation
Required Qualifications
- Active Registered Nurse (RN) license in Vermont or the ability to transfer license
- Home health and case management experience required
- OASIS certification or experience preferred
- Strong clinical assessment and documentation skills
- Ability to work independently in a home health setting
Preferred Experience / Skills
- Prior travel nursing experience
- No requested time off during the contract period
- Experience working in Central Vermont or similar settings
