Key Responsibilities
- Provide comprehensive physical therapy services to patients in a skilled nursing facility (SNF)
- Develop and implement individualized treatment plans based on patient needs
- Monitor patient progress and adjust treatment plans accordingly
- Collaborate with healthcare team members to ensure optimal patient care
- Document patient evaluations, treatments, and progress notes accurately
Required Qualifications
- Valid license as a Licensed Physical Therapist
- Minimum of 1 year of professional experience in a Skilled Nursing Facility (SNF)
Preferred Experience / Skills
- Experience working with diverse patient populations in a SNF setting
- Strong communication and teamwork skills
