LVN / LPN – Hospital Float (LTAC)

Contract | Nights (3x12 Hours, 7:00 PM – 7:30 AM) | Onsite | Every Other Weekend Required
Key Responsibilities
- Float across multiple units including long-term care households, subacute rehabilitation, memory care, pulmonary care, and ventilator units
- Assess patient status and implement clinical nursing interventions to modify symptoms and improve outcomes
- Develop and implement individualized patient care plans based on each resident's needs, routines, likes, and dislikes
- Provide care for patients with complex and chronic conditions including post-operative, post-stroke, post-cardiac, wound care, dementia, diabetes, and chronic respiratory conditions
- Manage and monitor patients requiring ventilator support, including responding to ventilator alarms, following specialized training protocols
- Adhere to strict time and attendance policies, including working every other holiday and observing facility blackout date guidelines
Required Qualifications
- Valid LPN/LVN license in Maryland or a Compact state (required at time of submission)
- Minimum of one (1) year of LPN/LVN experience
- Current ACLS and BLS certifications required
- Ability to float within the hospital across multiple units and levels of care, within scope of practice
- Must be available to attend all orientation days (Tuesday–Friday, day shift) as scheduled
- Professional references required
Preferred Experience / Skills
- Experience working in long-term care, subacute rehabilitation, pulmonary care, or ventilator units
- Familiarity with caring for patients with dementia, complex respiratory conditions, or ventilator dependency
- Strong organizational and leadership skills with the ability to thrive in a fast-paced environment
