Key Responsibilities

• Provide direct physical therapy services to patients in accordance with physician orders and established care plans
• Evaluate patient functional abilities and limitations to develop individualized treatment plans
• Implement therapeutic interventions to improve patient mobility, strength, and overall physical function
• Document patient progress, treatment outcomes, and any changes in condition in a timely and accurate manner
• Collaborate with interdisciplinary care team members to ensure coordinated and effective patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Iowa or eligibility for licensure
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Ability to commit to guaranteed hours: 36 hours per week for twelve-hour shifts, or 40 hours per week for eight to ten hour shifts

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Strong documentation and communication skills
