Contract | Full-Time | Onsite | Outpatient Clinic | Day Shift | 40 hours per week | Monday–Friday | Rotating schedule: Mon/Wed/Fri 7:00 AM–3:30 PM, Tue/Thu 10:30 AM–7:00 PM

Key Responsibilities

• Provide outpatient physical therapy services to adult, adolescent, and geriatric patient populations
• Manage a caseload of approximately 19–10 visits per day and up to 55 visits per week, including screening and caseload building
• Treat a broad range of orthopedic conditions including but not limited to ACL tears, rotator cuff repairs, disc herniation, total joint replacement, and generalized low back pain
• Treat neurological conditions including CVA, Parkinson's Disease, MS, Alzheimer's/Dementia, and degenerative diseases
• Conduct Fall Risk Assessments, Gross Motor Assessments, and CPS Cognitive Performance Scale evaluations
• Document patient care and progress using NextGen electronic health records system

Required Qualifications

• Active Physical Therapist licensure in the state of Illinois
• Minimum of 1 year of physical therapy experience
• Experience treating orthopedic and neurological patient populations
• Proficiency or ability to quickly learn NextGen EMR system
• Ability to adhere to a business casual dress code

Preferred Experience / Skills

• Experience with Fall Risk Assessments and Gross Motor Assessments
• Familiarity with CPS Cognitive Performance Scale
• Experience working across adult, adolescent, and geriatric populations in an outpatient setting
