Guaranteed Hours: 40 hrs. Special Requirements: • Department – BJC Home Health in Glen Carbon, Il., consists of a Multidiscipline Team, Geriatrics and middle age, no pediatric patients. • Patient Dx – Ortho, cardiac, neuro, pulmonary, Diabetes, other. • Case management skills, independent, time manage and organizational skill, patient centered goals, supervising PTA, Team Player, knowledge, professionalism, communication skills, flexibility, positive attitude, provide excellent quality care. This position is primarily week days but also involves participation in the weekend rotation. Previous home health experience strongly preferred.
