We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Snife Facility. Key Responsibilities:
• Assess, plan, coordinate, and implement patient care plans, including medication administration, wound care, and vital sign monitoring • Develop and maintain positive relationships with patients, families, and healthcare professionals • Conduct thorough patient assessments, including medical histories, physical examinations, and laboratory tests • Administer medications, vaccinations, and other treatments as prescribed by physicians • Provide education and support to patients and families on health promotion, disease prevention, and management of chronic conditions • Collaborate with the interdisciplinary team to develop and implement individualized care plans • Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes • Participate in quality improvement initiatives and contribute to the development of best practices
Qualifications:
• Current licensure as an LVN/LPN in the state of [State] • Minimum 1 year of experience in long-term care or acute care settings • Strong knowledge of medical terminology, anatomy, and physiology • Excellent communication and interpersonal skills • Ability to work effectively in a fast-paced environment and prioritize multiple tasks • Basic life support (BLS) certification
