Key Responsibilities
• Provide skilled occupational therapy services to clients in a home health setting
• Assess client needs and develop personalized treatment plans
• Document patient progress using electronic health records, including EPIC and Oasis
• Collaborate with interdisciplinary team to ensure quality patient care
• Visit up to six patients per day, maintaining appropriate productivity standards
• Ensure compliance with all facility and agency protocols and certifications
Required Qualifications
• State of Oregon occupational therapist license
• BCLS certification
• Minimum of 1 year of skilled home health experience
• Own reliable transportation for local travel within a 50-75 mile radius
Preferred Experience / Skills
• Experience with Oasis documentation system
• Familiarity with home health regulations and workflows

• No weekend requirements or on-call duties
• No float between units necessary

• Mileage reimbursement as per IRS standards
