Job Title

Key Responsibilities
• Provide occupational therapy services to adult patients in a home health setting.
• Manage a caseload of 4-5 patients per day with a focus on individualized care.
• Document services accurately using HomeCare HomeBase software.
• Collaborate with team members to coordinate patient care and ensure compliance.
• Occasional weekend work may be required; discuss specifics during the interview.
• Maintain timely communication and documentation to support smooth care delivery.

Required Qualifications
• Minimum of 1 year of recent skilled hospice experience within the last 3 years (inpatient experience not accepted).
• Current Washington state OT license.
• BCLS certification.
• Must have own transportation, as travel within a 100-mile radius may be required.
• Ability to visit up to 6 patients per day and meet productivity standards.

Preferred Experience / Skills
• Experience working in home health or hospice environments.
• Familiarity with documentation tools such as HomeCare HomeBase.
• Strong communication skills and reliable transportation.
