Key Responsibilities

• Provide skilled physical therapy services to adult patients in a home health setting.
• Manage a caseload of 4-5 patients per day, ensuring quality care and documentation.
• Conduct assessments and develop treatment plans tailored to individual patient needs.
• Maintain compliance with all licensing, documentation, and home health requirements.
• Collaborate with team members and utilize provided tools to ensure efficient service delivery.
• Occasional weekend visits may be required; on-call duty is not expected.

Required Qualifications

• Minimum of 1 year recent home health physical therapy experience (specializing in adult care).
• Valid Washington state license.
• Current BCLS certification.
• Experience with OASIS documentation.
• Own transportation required; reliable vehicle preferred.

Preferred Experience / Skills

• Prior experience working with home health agencies.
• Ability to work independently within territory constraints.
• Strong communication and time management skills.
