Schedule / Job Type

• Full-time – 40 hours per week, direct contract, on-site

Key Responsibilities

• Develop and implement personalized treatment plans to restore function, improve mobility, and relieve pain for residents.
• Assess patient needs, set achievable goals, and modify care plans under physician guidance.
• Collaborate with interdisciplinary teams to evaluate progress and adjust therapy strategies.
• Educate residents and their families on therapy methods and progress.
• Document patient assessments, treatment plans, and progress in compliance with regulations.
• Supervise and provide consultation to therapy assistants and support staff.

Required Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in Montana.
• Valid CPR certification.
• At least six months of experience in a long-term care or skilled nursing environment preferred.

Preferred Experience / Skills

• Experience with MDS procedures and care planning in skilled nursing facilities.
• Knowledge of HIPAA, federal, and state regulations concerning therapy documentation.
• Ability to communicate effectively with residents, families, and healthcare professionals.
• Strong organizational and confidentiality skills.
