Key Responsibilities

• Provide occupational therapy services to patients in a skilled nursing facility (SNF) setting.
• Assess patient needs and develop individualized treatment plans.
• Implement therapy interventions to improve or maintain patient functional abilities.
• Document patient progress and maintain accurate records in compliance with facility standards.
• Collaborate with interdisciplinary team to coordinate patient care.

Required Qualifications

• Valid occupational therapist license or eligibility for temporary/DC license.
• Professional experience working in a skilled nursing facility.
• Strong assessment and treatment plan development skills.
• Excellent communication and documentation skills.

Preferred Experience / Skills

• Experience working with diverse patient populations.
• Ability to adapt therapy plans to meet individual patient needs.
• Knowledge of current occupational therapy practices and standards.
