Key Responsibilities

• Provide patient care in a skilled nursing facility with a bed capacity of 66.
• Monitor and treat patients with conditions such as CHF, dementia, DM, CKD, COPD, dysphagia (peg tube), urinary retention (Foley catheters), and wounds.
• Follow patient-specific care plans and document care using PCC electronic medical records.
• Collaborate with a team of nurses to ensure quality care and maintain appropriate patient ratios (currently 22-25:1).
• Follow facility-specific orientation procedures by shadowing another nurse for one shift.

Required Qualifications

• Valid Registered Nurse (RN) license.
• Experience in a long-term care or skilled nursing facility preferred.
• Ability to work 12-hour shifts (either 8s or 12s available).
• Basic proficiency with electronic health records, specifically PCC.
• Willingness to work every other weekend and holiday.

Preferred Experience / Skills

• Prior experience managing patients with chronic conditions such as CHF, COPD, or wounds.
• Strong communication and teamwork skills.
