Key Responsibilities

• Provide outpatient physical therapy services to geriatric patients in a home health setting within a designated territory.
• Conduct assessments and develop treatment plans tailored to individual patient needs.
• Document patient progress and communicate effectively using electronic health records.
• Ensure timely completion of all documentation and maintain compliance with agency and healthcare standards.
• Participate in on-call rotation, providing services on weekends as scheduled.
• Support quality care delivery with flexible territory coverage within a 60-mile radius.

Required Qualifications

• Valid state license as a Physical Therapist in Washington.
• Minimum of 1 year of relevant experience in home health care.
• Proficiency with HCHB (Homecare Homebase) and OASIS documentation for SOC.
• BLS certification.
• Active driver’s license and reliable personal transportation.
• Ability to work independently with timely communication and documentation.

Preferred Experience / Skills

• Experience with hospice and home health documentation.
• Familiarity with travel within territory and flexibility in scheduling.
• Prior home health or geriatric care experience preferred but not required for first-timers.

• Mileage reimbursement at IRS standard rate.
• Travelers must submit RTO requests at application; on-call rotation required every 3rd to 4th weekend.
• Former employees must be separated for at least 6 months before reapplying as travelers.
