Key Responsibilities
- Provide home health nursing care to patients in their residence within Central Vermont.
- Conduct assessments and develop individualized care plans in compliance with agency protocols.
- Manage patient caseloads, ensuring quality and timely care delivery.
- Document patient interactions and treatment plans accurately, including OASIS submissions.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
Required Qualifications
- Current Registered Nurse (RN) license.
- Valid OASIS experience.
- Home health and case management experience.
- Ability to work independently in a home health setting.
Preferred Experience / Skills
- Previous travel nursing experience is highly preferred.
- Strong organizational and communication skills.
- No requested time off during the duration of the contract.
