- Full-time, Days, 8-hour shifts, 5 days per week
Key Responsibilities
- Develop and coordinate individualized care plans for hospice patients in collaboration with physicians, patients, and families.
- Provide ongoing assessment of patients' clinical, functional, and emotional status, adjusting care plans as needed.
- Communicate and collaborate with an interdisciplinary team to ensure comprehensive patient care.
- Educate patients and caregivers on care techniques and symptom management, including end-of-life care.
- Document patient evaluations, responses to treatments, and progress toward care goals.
- Participate in case conferences and ensure care plans are reviewed and recertified regularly.
Required Qualifications
- Valid RN license in Vermont, New Hampshire, or a valid compact license.
- Graduate of an accredited nursing school.
- Experience in Medical/Surgical, Oncology, or hospice care preferred.
Preferred Experience / Skills
- Prior hospice care experience is advantageous.
- Strong communication and compassionate patient care skills.
