Key Responsibilities
• Evaluate patients and develop individualized treatment plans in the home environment, coordinating care with multidisciplinary teams.
• Provide direct physical therapy services based on established care plans and physician orders.
• Apply evidence-based practice guidelines and ensure compliance with Medicare regulations and Conditions of Participation.
• Manage CMS-required assessments and collaborate with the Quality team for necessary adjustments.
• Perform supervised interventions, including wound care, as permitted under the physical therapy practice act.
• Document all services, adverse events, and communications accurately in the medical record.
• Participate in multidisciplinary conferences to establish and adjust patient care plans.
• Contribute to Quality Assurance and Performance Improvement initiatives.
• Oversee services provided by the Physical Therapist Assistant and supervise other staff as required.
• Maintain licensure, participate in continuing education, and adhere to ethical standards.

Required Qualifications
• Bachelor's degree or higher in Physical Therapy from an accredited program.
• Valid license to practice as a Physical Therapist in Montana.
• Current driver’s license and auto insurance.
• Excellent documentation and communication skills.
• Ability to manage a flexible schedule and travel efficiently within assigned areas.

Preferred Experience / Skills
• At least two years of previous physical therapy experience.
• Knowledge of home health care regulations and CMS assessment protocols.
