Key Responsibilities

• Provide skilled physical therapy services to home health patients in East Portland and surrounding areas.
• Evaluate patient needs and develop appropriate treatment plans.
• Conduct therapy sessions, monitor progress, and adjust treatments as necessary.
• Document patient care accurately using electronic health records such as EPIC and Rover.
• Collaborate with healthcare teams to ensure optimal patient outcomes.
• Maintain and demonstrate compliance with all licensing, certification, and agency policies.

Required Qualifications

• Must have at least 1 year of skilled home health experience.
• Valid license to practice as a Physical Therapist in the state of Oregon.
• BCLS certification.
• Experience with Oasis documentation system.
• Own transportation for travel between patient locations.

Preferred Experience / Skills

• Previous experience in home health settings.
• Ability to navigate and utilize health IT systems like EPIC and Rover.
• Flexibility to accommodate occasional travel within a 60-mile radius.
