- Full-time, Contract (3 shifts per week, days shifts, 12-hour shifts)
Key Responsibilities
- Provide in-home patient care for individuals requiring health management and support
- Perform assessments and develop care plans aligned with patient needs
- Administer treatments and medications as prescribed by healthcare providers
- Document patient care activities using OASIS documentation standards
- Collaborate with the healthcare team to coordinate patient care and discharge planning
- Ensure compliance with all relevant healthcare regulations and quality standards
Required Qualifications
- Active Registered Nurse (RN) license in Vermont
- Previous experience in home health and case management required
- OASIS experience is mandatory
- Strong clinical skills and attention to detail
- Ability to work independently and handle complex patient cases
Preferred Experience / Skills
- Previous travel nursing experience in home health settings
- Excellent communication and organizational skills
- No requested time off for the duration of the contract
