LVN / LPN LTC/SNF

Job Title

• LVN / LPN - Long-Term Care / Skilled Nursing Facility

Schedule / Job Type

• Full-time, days shifts, 3 shifts per week, 12.5 hours per shift

Key Responsibilities

• Provide vital sign assessments and medication administration to residents in a long-term care or skilled nursing environment.
• Monitor and document residents' conditions, reporting any changes to the healthcare team.
• Assist with daily living activities to promote resident comfort and well-being.
• Follow all facility protocols and safety procedures to ensure quality patient care.
• Collaborate with interdisciplinary teams to develop and implement care plans.

Required Qualifications

• Valid LVN or LPN license in the state of New Mexico.
• Current CPR and First Aid certifications.
• Minimum of 1 year of experience in LTC/SNF settings preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills

• Experience with electronic health records (EHR) systems.
• Knowledge of state and federal healthcare regulations.
