Key Responsibilities

• Provide patient care in long-term care and skilled nursing facility settings.
• Administer medications and monitor patient health status.
• Follow care plans and document patient information accurately.
• Collaborate with healthcare team to ensure quality patient outcomes.
• Ensure compliance with safety and healthcare regulations.

Required Qualifications

• Valid and active LVN or LPN license.
• Experience in long-term care or skilled nursing facilities preferred.
• Ability to work independently and as part of a team.
• Effective communication skills and attention to detail.
• Basic understanding of healthcare documentation standards.

Preferred Experience / Skills

• Prior experience working in LTC or SNF environments.
• Familiarity with electronic health records (EHR) systems.
• Bilingual abilities are a plus.
