Key Responsibilities

• Provide physical therapy assistance to patients in a home care setting.
• Collaborate with the physical therapy team to develop and implement treatment plans.
• Document patient progress and communicate effectively with supervising therapists.
• Adhere to all safety and compliance protocols while delivering patient care.
• Ensure timely and professional service delivery in accordance with company standards.

Required Qualifications

• Valid PTA license and Connecticut state license required before start date.
• Minimum of 1 year of home care experience.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in documentation and treatment planning.

Preferred Experience / Skills

• Experience working in a home health or home care environment.
• Knowledge of best practices in physical therapy assistance.
• Certification in CPR or other relevant clinical training preferred.
