Key Responsibilities
- Provide social work services to hospice patients and their families, including emotional support, counseling, and resource coordination.
- Assist with pain and symptom management in collaboration with healthcare teams.
- Support patients in various care settings such as private residences, skilled nursing, assisted living, and foster homes.
- Document patient interactions and coordinate care plans according to regulatory guidelines.
- Engage in bereavement support and connect families to community resources as needed.
Required Qualifications
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure.
- Valid driver's license and auto liability insurance.
- Healthcare Provider Level BLS certification.
Preferred Experience / Skills
- Experience as a social worker in a hospital or healthcare setting.
- LCSW (Licensed Clinical Social Worker) licensure preferred.
