Schedule / Job Type

• Full-time, days shift, 8-hour shifts, 5 days per week

Key Responsibilities

• Provide quality nursing care to residents in a long-term care or skilled nursing facility.
• Monitor and document patient health status using the facility's EMR system.
• Assist with patient assessments, medication administration, and care planning.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Adhere to facility policies and state regulations to maintain compliance.
• Support patient comfort and safety during your shift.

Required Qualifications

• Valid LVN / LPN license in the state of Minnesota.
• Basic life support (BLS) certification.
• Prior experience in LTC or SNF setting preferred.
• Ability to work independently and as part of a team.
• Excellent communication and patient care skills.

Preferred Experience / Skills

• Experience with the Matrix EMR system.
• Familiarity with general LTC patient populations and short-term care.
• Ability to work every other weekend and manage holiday shifts.
