Full-time, Night shifts, 5 shifts per week, on an 8-hour schedule

Key Responsibilities

• Provide nursing care to residents in a Skilled Nursing Facility specializing in long-term and short-term care.
• Monitor patient health, administer medications, and document care activities using electronic medical records (EMR) system.
• Assist with patient assessments and coordinate with interdisciplinary team to ensure quality care.
• Support resident needs during night shifts, ensuring comfort and safety.
• Follow facility-specific protocols and adhere to scheduled shift requirements, including at least every other weekend and half holiday coverage.
• Maintain compliance with facility policies regarding patient care and documentation.

Required Qualifications

• Valid LVN or LPN license in the state of Minnesota.
• Experience working in long-term care or skilled nursing facilities is preferred.
• Ability to operate electronic medical records (EMR) systems, preferably Matrix.
• Strong observation and communication skills.
• Willingness to work night shifts and weekend rotations.

Preferred Experience / Skills

• Familiarity with general LTC patient populations and short-term care.
• Experience working with diverse patient conditions and diagnoses.
• Ability to handle a patient load with staffing ratios of 1:20 during day/evening shifts and 1:40 at night.
