Job Title

Schedule / Job Type

• Day shifts, full-time, 5 days per week, 8-hour shifts

Key Responsibilities

• Provide occupational therapy services to home health patients within a designated territory
• Document patient assessments and treatment plans accurately and timely using HCHB EMR system
• Ensure compliance with all documentation and communication standards
• Conduct assessments and implement treatment strategies tailored to geriatric patient needs
• Collaborate with care teams to support patient recovery and functional improvement
• Follow on-call schedule as required, including weekend rotations

Required Qualifications

• Minimum of 1 year of relevant experience in home health OT
• Recent experience with at least 6 visits daily and 30 points weekly for productivity
• Proficiency in HCHB (Homecare Homebase) electronic medical records system
• Valid occupational therapy license in the state of Washington, in hand
• BLS certification

Preferred Experience / Skills

• OASIS experience, particularly for staff completing SOC in home health
• Flexibility with territory and travel up to a 60-mile radius from the office
• Experience working with geriatric populations in home health settings
