Key Responsibilities

• Provide licensed practical nursing care in a long-term care or skilled nursing facility setting.
• Cover CNA duties as needed to support patient care.
• Work scheduled weekends, including every other weekend, with flexibility to meet unit needs.
• Complete bi-weekly schedules with potential variations of 4x8s or 5x8s shifts.
• Maintain accurate documentation of all clinical activities, including work history and facility details.
• Adhere to facility protocols and ensure compliance with all licensing and safety standards.

Required Qualifications

• Valid NY state LVN/LPN license or proof of application with applied date.
• Minimum of 1 year of experience in a LTC or SNF setting.
• AHA BLS certification.
• Ability to cover CNA duties as needed.

Preferred Experience / Skills

• Experience with IV administration is a plus.
• Familiarity with electronic medical records systems and facility operations.
• Reliable transportation, personal or rental vehicle, with all-season or snow tires from November to April.

Additional Information

• Assignment duration: 16 to 32 weeks.
• Schedule includes no call or self-scheduling; shifts are assigned.
• Travel rate: $75 per hour; local rate: $65; radius: 51-100 miles.
• The facility requires all employment history information, including facility details and bed count, to be complete at submission.
• Orientation sessions are bi-weekly on Mondays, next available start date is 3/2.
• All work gaps over 90 days should be explained.
