Key Responsibilities
- Manage the active care and treatment of patients, collaborating with healthcare providers and interdisciplinary teams to develop individualized care plans.
- Facilitate patient referrals to specialized clinics, review referral records, and track referral progress to ensure timely care.
- Serve as the primary clinical contact for patient care coordination, proactively identifying and resolving obstacles to facilitate seamless healthcare delivery.
- Perform staff nursing duties including patient assessments, documentation, education, and administration of medications and therapies in various clinical settings.
- Maintain clinical competencies, safety standards, and infection control practices to ensure a safe and effective healthcare environment.
- Promote teamwork and effective communication within the healthcare team, contributing to department and organization-wide improvement initiatives.
Required Qualifications
- Current, unrestricted Registered Nurse license in Alaska or another U.S. state; must have applied for Alaska license if necessary.
- Graduate of an accredited school of nursing; Bachelor's degree preferred.
- Basic Life Support (BLS) certification from the American Heart Association.
- Clinical competency within 3 months of hire and every three years.
- Knowledge of nursing process, safety, infection control, and excellent assessment and communication skills.
- Ability to multi-task, work independently, and collaborate effectively in a fast-paced environment.
Preferred Experience / Skills
- Prior experience in patient care coordination or case management.
- Strong problem-solving skills and critical thinking.
- Ability to teach and mentor team members as appropriate.
