Job Title

Schedule / Job Type

• Full-time, day shifts, 5 days a week (8:00 AM – 4:30 PM)

Key Responsibilities

• Provide outpatient physical therapy services to a busy clinic setting
• Conduct assessments, establish discharge plans, and set short-term goals
• Take vital signs and monitor patient progress
• Implement home exercise programs and assist with lifting and transfers
• Document patient care using Cerner charting system
• Support facility needs within scope, including potential float responsibilities

Required Qualifications

• Minimum of 1 year of outpatient therapy experience
• Valid Illinois state license in good standing
• BLS certification
• Active or pending IL license (must have license in hand at submission)
• Experience taking vital signs and working in outpatient clinics

Preferred Experience / Skills

• Experience with assessment, discharge planning, and goal setting
• Knowledge of signs and symptoms across various types of abuse
• Ability to manage time efficiently and work independently
