Key Responsibilities
- Provide direct care to residents in long-term care or skilled nursing facility settings
- Administer medications and treatments as prescribed
- Monitor patient vital signs and report any changes to the healthcare team
- Document patient care accurately and thoroughly
- Assist with daily living activities and ensure residents’ comfort and well-being
Required Qualifications
- Valid LVN or LPN license in the state of New Hampshire
- Proven clinical experience in a long-term care or skilled nursing facility
- Ability to work independently and as part of a healthcare team
- Strong communication and documentation skills
Preferred Experience / Skills
- Previous experience working in LTC or SNF environments
- Basic life support (BLS) certification
- Familiarity with electronic medical records systems
