Contract | Full-Time | Days | Monday–Friday (flexible scheduling options available, including Tuesday–Saturday or Sunday–Thursday)

Key Responsibilities

• Provide physical therapy assistant services across both Skilled Nursing Facility (SNF) and Home Health (HH) settings, rotating approximately 50% between each on a week-to-week basis

• Deliver patient care in accordance with established treatment plans under the supervision of a licensed Physical Therapist

• Meet productivity expectations: 88% productivity in the SNF setting (group and concurrent time counted in full) and 95% productivity in the Home Health setting (inclusive of drive time and documentation time)

• Accurately document patient care and progress using electronic documentation systems

• Travel between patient locations within the Home Health setting, with mileage and drive time compensated between locations and patient-to-patient

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Iowa or ability to obtain prior to start

• Ability to work effectively in both SNF and Home Health environments

• Proficiency or ability to learn electronic documentation platforms (Net Health and HCHB)

• Ability to meet established productivity standards in both care settings

• Valid driver's license and reliable transportation for Home Health visits

Preferred Experience / Skills

• Prior experience in a Skilled Nursing Facility and/or Home Health setting

• Familiarity with Net Health and/or HCHB documentation software

• Comfort working independently in a Home Health environment with varying daily schedules
