Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs and goals
• Complete required clinical documentation accurately and in a timely manner in accordance with facility standards
• Collaborate with the clinical team to ensure continuity and quality of patient care
• Maintain compliance with all credentialing, licensure, and facility-specific requirements throughout the assignment

Required Qualifications

• Active Physical Therapist license in the state of Georgia (permanent license required; compact licensure alone is not sufficient)
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility credentialing and contract quality assurance requirements
• Must not have worked at this facility in a permanent or PRN capacity within the past year

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Familiarity with outpatient documentation systems and clinical workflows
