Case Manager, Days, 7H, 35H a week, 9a-5p with 1 hr lunch and every other weekend

MUST have:
Active NY License at TIME OF SUBMISSION

RN and BSN required

Must have at least 2 years of experience in Case Management

Must have discharge planning experience for patients on med-surg/ ICU units.

Experience in inpatient hospital setting medical units

Must be fully vaccinated against Covid, provide proof of vaccination at time of submission

NY PRI certification a plus but not required.

Inpatient medical units including assessment, identification of barriers to discharge, escalation, referrals to post discharge facilities and vendors (e.g Homecare, Rehab),engagement of patient and caregiver in the plan, communication and collaboration with multidisciplinary team . Clinical Reviews to payors (use of MCG guidelines) as required. Expected to be able to jump in quickly to support discharge planning

Orientation: 4 hr Fast Track training on system – Careport ACM, discharge planning concepts. Self-study modules with preceptor on unit.

Professional attire; No Scrubs.

Street parking.

Northwell will try to keep HCP at assigned facility but may be redirected based on Staffing Needs

Northwell does not allow use of personal PAPR equipment, if HCP fails fit test and uses PAPR equipment, the HCP will not eligible for assignment

In the event of inclement weather HCP is expected to be at work for any and all scheduled shifts. HCP will be able to arrive early and/ or bring the necessary items to stay late after shift if needed. Weather is NOT an acceptable excuse for not reporting as scheduled.
