**CANDIDATES MUST HAVE AT LEAST 1 PREVIOUS TRAVEL ASSIGNMENT**
tower 4 is neuro/tele with a 6 room SDU on the unit.
ACLS, BLS
– Level 1 experience required
– Must be proficient in Tele
– Will not take less than 13 weeks
No block scheduling requests, unless for Weekends Only (that will be considered, but not necessarily approved)
MILEAGE REQUIREMENT: Candidate must be at least 60 miles from ANY UH facility to qualify for Travel Rate. Candidates within 60 miles will qualify for Local Rate ($105). To ensure your candidate meets the requirements, please ensure candidate is not 60 miles from any of the following 3 facilities: UH Portage (Ravenna, OH), UH Elyria (Elyria, OH), , UH Samaritan (Ashland, OH), UH Conneaut, regardless of facility candidate will be working.
Facility asks the following –
1. RTO is due at submittal – any extra RTO after offer will be discussed with floor manager upon starting.
2. A candidate should only be submitted to maximum of one job at a time within all of UH.
Former perm employees must wait until after 6 months to be sent in for a position
Former travel can come back at any time
Fit Test completed on-site. Gentlemen must be clean shaven.
Per UH policy, candidates cannot request off more than 10 days, with no more than 5 consecutively requested off. Any RTO exceeding this requirement will immediately be removed and asked to trim down to meet this requirement before resubmitting.
