Key Responsibilities
- Provide skilled nursing care to patients in a home health setting.
- Develop and implement patient care plans in collaboration with the healthcare team.
- Monitor patient progress and adjust treatments as needed.
- Educate patients and families on health management and recovery.
- Document patient care activities accurately and thoroughly.
- Ensure compliance with all regulatory and safety standards.
Required Qualifications
- Valid Illinois Registered Nurse license or active application in process.
- Ability to produce a negative drug screening test, as required on a drug-free campus.
- Last four digits of Social Security Number and middle initial for submission.
- Willingness to work at least 12-hour shifts.
- Availability for requested time off documentation.
Preferred Experience / Skills
- Previous experience in home health nursing.
- Knowledge of Illinois healthcare regulations and standards.
