Job Title
• LVN / LPN - LTC/SNF

• Chamberlain, SD

Schedule / Job Type
• Full-time, 12-hour shifts, rotating weekends and holidays

Key Responsibilities
• Provide quality nursing care to residents in a long-term care or skilled nursing facility
• Assist with patient assessments, medication administration, and care planning
• Collaborate with interdisciplinary team to ensure patient needs are met
• Document patient information accurately in the electronic medical record
• Support residents with daily activities and monitor their health status
• Adhere to facility policies and safety procedures

Required Qualifications
• Valid LVN or LPN license in the state of South Dakota
• Current CPR/BLS certification
• Strong communication and patient care skills
• Ability to work independently and as part of a team
• Experience in LTC/SNF setting preferred

Preferred Experience / Skills
• Experience with dementia, Alzheimer’s, diabetes, or cardiac conditions
• Familiarity with EMR systems such as Matrix
• Ability to rotate between day and night shifts
• Flexibility to work weekends and holidays
