Key Responsibilities
• Provide skilled physical therapy services to residents in a long-term care setting
• Assess patient needs and develop individualized treatment plans
• Implement therapy procedures to improve mobility, strength, and functional capacity
• Document patient progress and update treatment plans accordingly
• Collaborate with healthcare team to ensure comprehensive care

Required Qualifications
• Valid State License as a Physical Therapist
• Proven experience in a clinical or long-term care environment
• Strong assessment and documentation skills

Preferred Experience / Skills
• Experience working in a skilled nursing facility or senior care environment
• Knowledge of rehab equipment and therapy techniques
