Registered Nurse LTC/SNF

Job Title: Registered Nurse - LTC/SNF Location: Concord, NH Schedule / Job Type: Full-time, Days Key Responsibilities: • Provide high-quality, patient-centered nursing care to residents in a long-term care or skilled nursing facility setting • Assess, plan, implement, and evaluate patient care plans to meet individual needs and goals • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans • Monitor and report changes in patient condition, including vital signs and medication management • Educate patients, families, and caregivers on health promotion, disease prevention, and management of chronic conditions Required Qualifications: • Current RN licensure in the state of NH • Experience working in a long-term care or skilled nursing facility • Strong assessment, planning, and implementation skills • Excellent communication and interpersonal skills • Ability to work effectively in a fast-paced, dynamic environment Preferred Experience / Skills: • Experience with electronic medical records (EMRs) and other healthcare technology • Knowledge of gerontological nursing principles and practices • Certification in gerontology or geriatrics (e.g. AGS, ACG, or CG)
